A. Gather the Required Information
Before you report a separation or retirement, gather this information:
Date of the separation or retirement
If this is a voluntary separation or retirement, a letter from the employee indicating the reason for the separation or retirement
If this is NOT a voluntary separation or retirement, the reason for the separation
B. Launch the Form
If you are a:
Manager: Find the person in the My Team list of your Manager Self Service home page.
Department administrator: Find the person in the company directory of your Departmental Administrator home page.
Click the green arrow next to the person's name and choose Separation or Retirement.
C. Fill Out the Form
Use this section for guidance on filling out the form.
Note: You may see a slightly different version of the form, depending on the reason the person is leaving.
1. Effective Date
Typically, enter the day AFTER the person's last work day in this position.
2. Employee Directed?
Choose the option that best describes if the separation or retirement was voluntary. The remaining items on the form will vary depending on whether the employee initiated the separation.
3. Reason for Separation
If this is NOT a voluntary separation or retirement, choose the option that most closely matches the reason for the separation
4. Additional Documentation
If this is a voluntary separation or retirement, click Add and then upload the required documentation.
5. Save For Later
To save the form and finish it later, click Save For Later.
6. Submit
When you have completed the form, click Submit.
