Major Declaration: This form is used to declare or drop a major. To access this form, log into MySlice, go to eForms to obtain the Major Declaration Form, complete the form and you will receive a confirmation via email when it's been approved. To remain or to become a Chemistry major, a student must satisfy either of the following two requirements: Earn a grade of C+ or better in General Chemistry lecture and laboratory courses (CHE106/107/116/117 or honors equivalents, or AP credit for CHE106/107/116/117) AND earn a grade of C or better in CHE 275 OR Earn a grade of A- or better in a General Chemistry lecture course (CHE106/116/109/119) taken at Syracuse University.
Minor Declaration: This form is used to declare a minor. Please email the completed form to chemistry@syr.edu to obtain a signature from the Department Chair.
Withdrawal: This form is used to withdraw from a class. When filling the form out, please include the class numbers and sections for both lecture and recitation. You can find this information in MySlice. This should be sent to your college advisor.
Petition to Faculty: This form is used whenever a student is requesting approval for something outside the normal program or course requirements. Completed petitions are reviewed by the Department Chair, who will make a decision whether or not to approve the petition. Please email the completed form to chemistry@syr.edu to obtain a signature from the Department Chair.Â
Independent Study: This form is used for students who wish to take CHE 490 (Independent Study). Please email the completed form to chemistry@syr.edu to obtain a signature from the Department Chair.
Departmental Transfer Petition: This form is for students who wish to receive transfer credit for courses that will apply to your major or minor. Please email the form and a syllabus (should be for the semester you are taking the course) to chemistry@syr.edu to obtain a signature from the Department Chair.
Liberal Arts Core Transfer Petition: This form is for students who wish to receive transfer credit for courses that will apply to their liberal arts core requirements. This needs to be completed online and will be reviewed by A&S Advising. You do not need a signature from the Chemistry Department.
Distinction in Chemistry: This form is for students who wish to graduate with a Degree in Chemistry with Distinction. To receive this degree, a student must graduate with a 3.4 GPA (both overall and in chemistry) and complete a thesis describing a significant research project. A completed Distinction Proposal Form must be submitted online in the semester prior to graduation. May graduates need to submit this form by February 1, December graduates need to submit this form by September 15.Â
Distinction in Biochemistry:Â This form is for students who wish to graduate with a Degree in Biochemistry with Distinction. To receive this degree, a student must graduate with a 3.4 GPA (both overall and in biochemistry) and complete a thesis describing a significant research project.
