A. Gather the Required Information
Before you request an administrative or research leave for a faculty member, the following application must be completed:

B. Launch the Form
- If you are a:
- Manager: Find the person in the My Team list of your Manager Self Service home page.
- Department administrator: Find the person in the company directory of your Departmental Administrator home page.
- Click the green arrow next to the person's name and choose Faculty Admin or Research Leave.
C. Fill Out the Form
Use this section for guidance on filling out the form.
1. Type of Leave
Choose the type of leave the faculty member is applying for, either:
- Administrative
- Research
2. Timing
Choose the semester (s) and year(s) the faculty member is requesting the leave for.
3. Funding
For each semester of the leave, indicate:
- If the faculty member is NOT requesting funding through SU
- The percentages of the various funding options
The responses should be consistent with the faculty member's leave application.
4. Contingent Funding
If this is a request for a research leave, indicate a contingent source of funding for the leave in case the intended source is not available as planned. For each semester of the leave, indicate:
- If the faculty member is NOT requesting funding through SU as a contingent option
- The percentages of the various contingent funding options
The responses should be consistent with the faculty member's leave application.
5. Benefits
For each semester of the leave, indicate:
- Whether the faculty member wants SU benefits during the leave
- For a research leave, whether the faculty member wants SU benefits as part of the contingency option
6. Attachments

7. Save For Later
To save the form and finish it later, click Save For Later.
8. Submit
When you have completed the form, click Submit Update.
D. Check the Status of Approvals
An administrative or research leave requires approval to take the leave and to return from the leave.
Important: The faculty member must approve BOTH the request to take the leave AND the request to return from the leave. They will find those requests in the Approvals tile of their Employee Self Service home page.
