Frevvo is an excellent application that allows you build and deploy your own forms. The form designer interface is simply drag-and-drop, making forms very easy to create. With a couple additional clicks, you push that form into production so that users can begin filling it out. It's simplicity is where it shines. However, Frevvo also contains advanced features below the surface that allow for rules, data validation, and more complex functionality.
