Grad students: Depending on the form, either the Grad School (304 Lyman, grad@syr.edu) or the Registrar’s Office (106 Steele Hall, registrar@syr.edu)

Types of Forms and Petitions:

 

Major/Minor Declaration petitions process:

o   If a student says they are interested in declaring a major or minor, try to have the DUG meet with them on the spot per chair’s policy:

§  If they aren't available, Prof. Schiff is first back up, followed by Prof. Catterall and last Prof. Hubisz. Goal is to never let a potentially interested student leave without meeting with someone

§  Last resort give them the DUG's email so they can contact him to set up meeting. Get name and inform him to expect to hear from them as well.


Transfer Credit Petitions:

o   Undergrad Students:

§  If an undergrad student gives you a transfer credit petition, unless department chair requests otherwise --- The DUG is the lead reviewer. Students need to have the cover petition filled out (each college has own cover petition) and they need a copy of the syllabus plus if possible, a course catalog description attached (he will not review or approve without a syllabus!) He’ll determine if he can approve or not, and then approved petitions need to be returned to the student to take it to their home college dean’s office next for approval.

o   Grad Students: Student should use petition to faculty form.

§  Grad students need to use a petition to the faculty form for transfer credit requests. They need to attach a copy of a transcript and the syllabi for every class they want to transfer. The number of credits they should request to transfer should be the amount that the SU equivalent is worth, and not necessarily the amount the class was worth at their prior institution.

§  Need to be signed by academic advisor (before research oral) or research advisor (after having passed research oral) followed by the DGS (Director of grad studies). Petitions for Graduate transfer credit should be sent to Kristina Ashley at the Graduate School, 304 Lyman Hall, grad@syr.edu.

 

Independent Study Petitions:

o   Grad Students:

§  They should fill out the form with the faculty sponsor they want to work with.

·         The student should fill out the top of the form and they should complete the bottom half with their sponsor.

·         Most grad students will do “course electives” unless they are using the PHY 690 to replace one of the standard required courses (rare)

·         They should check boxes for PHY 690 and in general, they should all be 3 credits. Ask Prof. Soderberg to approve requests for PHY 990, or any number other than 3 credits.

·         I always note the independent studies in the “notes” section of database for future reference, to see who the student has been working with or what fields they have shown most interest in.

§  The Faculty Sponsor and the student should both sign this form. Then, send it to the chair for their signature. After they sign and returns it to you, you should copy the form for the student’s academic file, and then email it to  registrar@syr.edu (the registrar’s office)

o   Undergrad Students:

§  They should fill out the form with the faculty sponsor they want to work with.

·         The student should fill out the top of the form and they should complete the bottom half with their sponsor.

·         Their academic advisor should be consulted as to if it is an elective or replacing a required course.

·         Can be 290 (BA credit only, won’t count toward BS) or more likely 490.

§  The Faculty Sponsor and the student should both sign this form. Then, the student needs to get their academic advisor’s signature. Then, send it to the chair for their signature. After thee sign and returns it to you, I recommend you save a copy for future reference. Then, contact the student to pick it up(or email it back to them). They will need to bring it to their home college Dean’s Office for a signature of approval, next. Or you can send it to their home college's email.

 

 

Graduate Student Research Oral Exams:

o   NOTE: This is NOT their oral defense, right before they graduate. This is their research oral exam, completed approx. their third year of study.

o   Grad students need to just give this form to their exam committee for completion after their exam. The committee fills out the whole thing and signs all the lines they need.

o   If the chair did not already sign the bottom line when it is turned in, you need to first solicit a signature from them.

o   Once they have signed it, make a copy for their academic file and then email it to the Graduate School, degreecert@syr.edu, with a note on top advising them to please enter the milestone in as “Qualifying Exam II”

o   NOTE once this is turned in, into the database for department records. Technically the faculty member who signs off on the research oral should be considered their advisor going forward. This is when we usually add the advisor to the student’s record in MySlice as well. Training is needed from RO,  but phy dept staff add grad advisors, not home colleges like ugrads. Once advisor is listed in MySlice they can view the student record info themselves.
