Grade Change

Only needed for students who are NA, NR, RM or WD or who have graduated. All other grade changes are done online. The Instructor/Department will complete this form and submit it to the Falk College Student Services Graduate Recorder. The Graduate Recorder will submit this form to the Registrar’s Office.

Grade Option

For registering a course as Pass/Fail or as an Audit. Falk College Student Services also needs a copy of this form for student records, please submit a signed copy to 330 Barclay Hall.

Incomplete Grade

For requesting an incomplete grade. An incomplete grade will calculate as an ‘F’ until an actual grade is submitted.

Independent Study Proposals

The Falk College Office of Student Services will submit the completed form to the Registrar’s Office after all signatures have been obtained.

For leaving for a semester or withdrawing from the University. A completed Leave of Absence form should be sent to the Falk College Office of Student Services at 330 Barclay Hall addressed to “ATTN: Graduate Recorder”.

Petition to Faculty – Transfer Credit

For transferring credits to Syracuse University. The Falk College Office of Student Services will submit the completed form to the Registrar’s Office after all signatures have been obtained.
